you’re a plumber, electrician, roofer, or general contractor,

Contractor Application Requirements:

Occupational License
Certificate of Liability Insurance listing Rebuilding Together Greater Florida as the certificate holder
Worker’s Compensation or exemption documentation
W-9 Form
Signed Sub-Contractors Agreement & Photo/Invoice Agreement (Download Below)
Completed ACH Form for payment (Download Below)
Signed Affidavit of Compliance with E-Verify and Employment Authorization Requirements (Download Below)
